How to Fill Up Online License Form?

After opening the form, first, select the zone without filling in anything else. Then wait a while and select District. Then wait a while again and select the VDC. And also select the zone above Captcha. Again, wait a while and select LICENSE ISSUE OFFICE. I am saying it again. Don’t fill out anything else until this is done.

This is to find out if the server is working while you are trying to submit the form. Refresh the Captcha until it shows a new Captcha. If the daily quota is still available, your form will go to the registration process, and you can finally save your form and also download it in PDF format.
